IMPORTANT

Every youth attending Fellowship Rouge Youth (FRY) must be registered using this form. If you are sending multiple youth under your care, please submit a unique registration for each.

Fellowship Rouge Youth (FRY) is collecting and retaining this personal information for the purpose of enrolling your child in our Friday youth program, and to inform parents and guardians about updates and announcements.

Information received is confidential and is being gathered for the purposes of serving your child while in the care of Fellowship Rouge Youth (FRY). Submitting your childs medical information is required to authorize Fellowship Rouge Youth leadership and volunteers to obtain expidited and unhindered assistance in the event of a medical emergency.

Consent

The safety of your child is our primary concern. Precautions will be taken for their well-being and protection.

I/we, the parents or guardians named below, authorize Mark/Lucy Jenkinson or one of the Fellowship Rouge Youth leadership team to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participants named above.

I/we, parents or guardians named below, undertake and agree to indemnify and hold harmless Fellowship Rouge Youth leadership, team members, volunteers, its Pastors and Elders from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of the Fellowship Rouge Youth, as well as of any medical treatment authorized by the supervising individuals representing the Fellowship Church Rouge Park.

This consent and authorization is effective only when participating in or traveling to events sponsored by the Fellowship Rouge Youth.

Fellowship Rouge Youth will email you a weekly FRY Parents Newsletter consisting of anouncements, calendar of events, and important updates. Please add [email protected] to your email safe list, so as to not miss a thing.

Photography Consent

We will not publish photos on social media or on our website, but we may occasionally use photos taken at FRY events in email newsletters shared with parents, Fellowship Rouge Youth teams, and Fellowship Church Rouge Park.
